Back in the day I lost control of my forklift inside the warehouse and ran the forks into the shipping supervisor's office. YIKES!
I had to give a detailed report of the accident to the safety director. I confessed I wasn't paying attention so the report lasted 5 minutes.

I never heard the end of jokes from my co-workers. lol!
