Here is a survey I made some time back modeled after Ray Comfort's style of witnessing.

I understand the intention of the survey, but it falls short on multiple accounts. For example, question 1 'Do you think you are a good person?' - an appropriate follow up question and one that should have been ask is 'based on what standard?' If we expect to challenge a nonbelievers worldview, then we must start asking the right questions.
